  • For state employees telecommuting for work, you have the option of scheduling a Mini Health Screening at the Healthwaves Tempe location that takes about 20 minutes.

    HIP POINTS

    • Earn 100 HIP points. 

    SCREENING ELEMENTS

    • NEW - Fasting Blood Test - Fasting for 10 hours REQUIRED. No food, water only.
      • Cholesterol and full lipid panel (HDL, LDL, VLDL, Triglycerides)
      • Blood sugar  (Glucose & Fructosamine)
    • NEW - Waist Circumference
    • Height
    • Weight
    • Blood pressure
    • Prostate Specific Antigen screen (PSA) for men age 40 and older
    • Hemoglobin A1C screen (for diabetics) 
    • Bone Density Screening for women age 40 and older
    • Osteoporosis: Free for women 40 and older. $35 copay for women under age 40.

    RESULTS

    • The fasting screening requires a blood draw which is evaluated in a lab.
    • Individual, confidential results will be mailed to each participant within a week of blood draw. If you did not receive your results in the mail, please call Healthwaves 480-968-1886.
    • A copy of the test results can be sent to the participant’s physician (participants need to provide physician contact information at the event).

    COST

    • Main screening components are FREE one time per year.
    • Costs for optional tests are listed above.

    LENGTH

    • Each screening takes 10 to 15 minutes
    • Total event time is based on the size of the work site

    PARTICIPATION

    • Minimum of 25 participants required (exceptions may be considered for rural locations).
    • Spouses, dependents and retirees are not eligible

    MAMMOGRAPHY SCHEDULE

    This screening qualifies you for 100 HIP points. 

    SCREENING ELEMENTS

    • Mammogram
    • 3-D (cost)

    SCREENING RESULTS

    • Confidential mammogram reports (results) will be sent to the participant and the physician they designate
    • Patients must provide the contact information if they want the report sent to their physician

    COST

    • Participants will need to show their Benefit Options insurance card at the time of their appointments.
    • MOM will directly bill the Benefit Options insurance.
    • EPO and HSA Members do not pay a copay at this screening it is covered 100%.
    • PPO members must meet the $500 deductible before this screening is covered at 100%.

    LENGTH

    • Each screening takes approximately 10 to 15 minutes

    PARTICIPATION

    • A minimum of 20 participants is preferred
    • Appointments are required to participate 480-967-3767.
    • Employees not on the State's insurance plan may participate, but other insurance copays may apply.
    • Spouses, dependents and retirees are not eligible.

    PROVIDER

    • Mobile Onsite Mammography (MOM) provides mammography (Breast Cancer) screenings.
    • The MOM mobile travels to perform mammography screening at worksites across Arizona.

    PROSTATE CANCER SCREENING SCHEDULE

    HIP POINTS

    This screening qualifies you for 100 HIP points. 

    SCREENING ELEMENTS

    • Prostate-specific antigen (PSA) blood test
    • Digital rectal exam (DRE)
    • Testicular exam
    • Physician consultation

    SCREENING RESULTS

    • Results are immediate and will be discussed with patients by the physician. 
    • The PSA blood test results will take 7-10 days, and patients will receive a call about their results.

    COST

    • FREE for Benefit Options health plan members (including spouses) and benefits-eligible employees not taking State insurance.
    • Participants will need to show their Benefit Options insurance card at the time of their appointments.
    • POP will directly bill the Benefit Options insurance for UnitedHealthcare and BCBS.  ADOA will pay for the cost for Aetna and CIGNA members, and eligible employees that are not enrolled in the medical plans.
    • This screening does not have a copay.

    LENGTH

    • Each screening takes approximately 10 to 15 minutes.

    PARTICIPATION

    • A minimum of 20 participants is preferred
    • Appointments are required to participate 480-964-3013
    • Screening events are scheduled for a minimum of 6 hours
    • Retirees are not eligible

    PROVIDER

    • Prostate Onsite Project (POP) provides prostate cancer screenings.
    • The POP mobile unit travels to perform prostate exams at work sites across Arizona.

    Free Hearing Screenings

    Dates and Times

    November 5 - 8 am - 4:30 pm

    Registration required. Register using this link.

    15-minute hearing screenings include:

    • Pure-tone test for key speech frequencies
    • Tympanogram for middle ear function
    • Ostoscopic evaluation for outer ear issues such as cerumen (ear wax) or other problems

    Important HIP Points Note:

    If you receive a hearing screening from a provider other than ASU, please send an email to [email protected] , with an EOB or document from the provider indicating the date of the screening and include your EIN. Please do not send detailed reports from the screening exam.
