Mini-Health Screening Overview

HIP POINTS
  • Earn 1,000 HIP points for attending a Healthwaves Screening. 
SCREENING ELEMENTS
  • Fasting Blood Test - Fasting for 8-10 hours REQUIRED. No food, water only.
    • Cholesterol and full lipid panel (HDL, LDL, VLDL, Triglycerides)
    • Blood sugar (Glucose & Fructosamine)
  • Height, waist circumference, weight
  • Blood pressure
  • Prostate Specific Antigen screen (PSA) for men age 40 and older
  • Hemoglobin A1C screen (for diabetics) 
  • Bone Density Screening for those age 40 and older
RESULTS
  • Individual, confidential results will be mailed to each participant within a week of the blood draw. If you did not receive your results in the mail, please call Healthwaves at 480-968-1886.
  • You may request an additional copy of your results to share with your physician.
COST
  • Main screening components are FREE one time per year.
  • Costs for optional tests may be paid directly by Healthwaves by cash, check or charge.
LENGTH
  • Each screening takes 10 to 15 minutes.
  • Total event time is based on the size of the work site.
PARTICIPATION
  • Benefits-eligible employees.
  • Spouses, dependents, and retirees are not eligible.
