EMPLOYEE COST

• All program costs paid by State of Arizona for each participant.
• This program requires the employee to be enrolled in a State medical plan.

COMPLETION DEFINITION

• 2000 HIP points earned after completing the program.

PARTICIPANT ELIGIBILITY

• Benefits-eligible employees, spouses and dependents age 18+.
• Must be enrolled in State medical plan.
