Free Screenings Year-Round

• Check our free screening provider list. 
• To receive free screening, tell the provider you are a State of Arizona employee.

Tests Performed

• A pure-tone test to determine if hearing levels are at or below the screening levels at key speech frequencies.
• A tympanogram measurement to determine middle ear function.
• An otoscopic evaluation will determine if outer ear issues, such as cerumen (earwax), or other problems, need attention.

Earn HIP Points
You may earn 100 HIP points toward the $200 annual reward. After your screening, visit the Virgin Pulse site, Home> Rewards> How to Earn> Participation> Hearing Screening, and complete the Hearing Screening Attestation Form.
