• Event Request Form - Agency Wellness Staff only 
• For multiple requests, please complete a separate form for each event. 
• Please submit requests 4-6 weeks prior to the requested event date to allow time for processing.

HIPSTER is our program to encourage wellness liaisons to learn and grow by promoting HIP in their agencies. 

The criteria include:

• Host HIP overview meetings.
• Conduct screenings and activities.
• Support HIP and Benefit Services communications.
