Starting in 2024, Assured Imaging will be our provider for on-site mammograms. You will still be able to use SimonMed-MOM for imaging, and you can obtain prior records from them.

SCREENING RESULTS

• Confidential mammogram reports (results) will be sent to the participant and the physician they designate.
• If patients want the report sent to their doctor, be sure to bring the necessary contact information. 

COST

• Participants will need to show their state medical insurance card at the time of their appointments.
• Your insurance carrier will be billed directly. 
• This is a preventive service covered at 100%.

LENGTH

• Each screening takes approximately 10 to 15 minutes

PARTICIPATION

• Appointments are required to participate. Click the event link above.
• Employees not on the State's insurance plan may participate, but other insurance copays may apply. 
• Retirees are not eligible.
